Sports Physicals

Effective July 1, 2024
All students wishing to try out for a sport in Middle School or High School must have a Preparticipation Physical Evaluation Physical Examination Form (Sports Physical) and a Preparticipation Physical Evaluation Medical Eligibility Form on file, which must be signed off by the school doctor. The clearance process can take 7-10 days.
In addition, a Preparticipation Physical Evaluation History Form must be completed by the parent/guardian. A Sports Physical is good for 365 days.Next, a NJ Department of Education Health History Update Questionnaire is required for the sport your child tries out for each season. Please access the parent portal or contact the school’s main office to find out the date the school has on file for your child’s last SPORTS physical.
Parents/Guardians and student-athletes must complete the FamilyID registration.

PLEASE BE AWARE OF YOUR TIME FRAME. THIS PROCESS, IF NOT COMPLETED IN A TIMELY MANNER, CAN PREVENT THE ATHLETE FROM TRYING OUT AND/OR PLAYING TIME. SIX (6) PRACTICES MUST BE COMPLETED TO PARTICIPATE IN A SCRIMMAGE/GAME.
