Seniors must fill out The Parking Policy and Agreement Form before a parking tag is issued. Please use this LINK.
BEFORE you begin this form, make sure you have the following:
1. Driver's License (or Permit--ONLY for Senior painted spots)
2. Registration of the vehicle you will be driving (the registration must be either in your name or your parent/guardian's name)
3. Automobile Insurance card
PLEASE MAKE SURE ALL ITEMS ARE CURRENT AND NOT EXPIRED!
