In order to report allegations of Harassment, Intimidation, or Bullying (HIB),
Fill out one of the following forms

Once completed, the form can be submitted in person or electronically.  Electronic submissions can be completed at the link below.  If you prefer to remain anonymous, leave the section blank on HIB 338 Form under the section titled Individual Completing Form and submit using an alternative Google account when accessing the form because it will capture the name associated with the email address.
