    Per the State of New Jersey and the Scholastic Student-Athlete Safety Act
    With the passage of the Scholastic Student-Athlete Safety Act, New Jersey has set the standard by requiring that thorough evaluation be conducted for silent cardiac abnormalities that may lead to the sudden death of a student-athlete. The below requirements are mandated by the state of New Jersey, without exception to participate in athletics for the 2021-2022 school year.

    Requirements for Participation in Athletics
    A student-athlete would be eligible for participation in athletics ONLY IF they meet ALL of the following criteria:

    1. Parent/Guardian and student-athlete must complete the FamilyID registration form and submit to Lacey Township Athletics by the registration deadline.
    2. Preparticipation Physical Exam Form (page 3 of this document) must be printed and submitted to the school after it is completed by a health care provider who:
      • is a licensed physician, advanced practice nurse or physician assistant; and
      • completed the Student-Athlete Cardiac Assessment Professional Development Module
      • The school doctor still has to review and sign-off on your physical once the form is handed in
    3. Health History Update Questionnaire (click here) must be printed and submitted to the school after it is completed by a health care provider who:
      • is a licensed physician, advanced practice nurse or physician assistant; and
      • completed the Student-Athlete Cardiac Assessment Professional Development Module

    *Please note that either #2 or #3 is mandatory for medical clearance and athletic participation, depending when the student/athlete's last full yearly physical was completed.

    Because the Preparticipation Physical Exam Form and the Health History Update Questionnaire (pages 3 & 4) require the signature of a health care provider, you must print these pages and then submit them to the school to be approved by the board-approved school doctor.

    • The first 2 pages will be filled out by the parent/guardian. 
    • The last 2 pages are to be filled out by the medical provider and stamped. The provider must have completed the Cardiac Assessment Professional Development Module to complete the sports physical.
    • The school physician will need to sign off on all physicals before a student may participate.

    Health History UPDATE QUESTIONNAIRE:
    To participate on a school-sponsored interscholastic or intramural athletic team or squad, a health history update questionnaire completed and signed by the student’s parent or guardian. This form needs to be submitted prior to the first day of official practice.
