TECHNOLOGY AGREEMENT K-12
As a student user of the Lacey
Township School District
technology facilities, I agree to the following rules:
- I agree to use the Lacey
Township School District’s
technology facilities only for the purposes that support the District’s
curriculum.
- I agree to use Lacey
Township School District’s
technology facilities in accordance with any software license agreements,
copyright restrictions, or any applicable local, state, or federal laws
that apply.
- I agree to use my privileges
to access technology resources only with the guidance and direction of my
teacher.
- I will respect the privacy of
information stored in Lacey Township
School District’s technology
facilities. I will not attempt to view or change information belonging to
another person, nor enter any restricted area.
- I will only use permitted
software owned by the Lacey Township
School District and abide by
copyright laws that apply.
- I will not share my password
with anyone. If I know of anyone attempting to use passwords other than
their own, I will report them to my teacher.
- I will not save or install
software on any of the Lacey Township
School District’s technology
facilities.
- I understand that vandalism
of hardware, software, or data will not be tolerated.
- I will not share personal
information (such as my addresses, telephone numbers, parents’ work
information, or the name and location of the school, etc.) or use e-mail
without my teacher’s specific permission.
- I will tell my teacher and
parents immediately if I see any information on-line that makes me feel
uncomfortable.
- I will never intentionally
create any documents that are offensive, or would hurt anyone in any way.
- Should I violate this
agreement, I understand that the consequences of
my actions may include detention, suspension or expulsion from
school, removal from an elective technology course, the loss of
technology privileges and or other discipline in accordance with the Lacey
Township School District’s
Student Parent Handbook for the relevant academic year.
- Should I violate this
agreement, I understand that the consequences of my action may also
include the Lacey Township Board of Education pursuing a civil legal
action against me as well as my parents/guardians and that if a civil
legal action is brought, the Board of Education will seek compensatory
damages, punitive damages (if warranted), and costs of such litigation
including but not limited to, investigation expenses, exerts fees, and
attorney’s fees.
- Should I violate this
agreement, I understand that my actions my also be reported to appropriate
law enforcement officials. I also understand that criminal penalties for
such acts perpetrated against the school district as a municipal
government entity include a mandatory sentence of imprisonment as well as
other criminal penalties.
- This agreement remains in
force as long as I am using the Lacey
Township School District’s
technology facilities during the current school year.
PARENT OR GUARDIAN: As the parent or guardian
of this student, I have read and agree to the terms and conditions of this
agreement. I understand that the student named below is expected to use
the resources according to the specified guidelines. I have discussed
these guidelines with my child. I recognize that it is impossible for the
Lacey Township
School District to completely
control information available through the Internet. I hereby give my
permission for him/her to use the Internet at school and certify that the
information contained on this form is correct.

New Jersey Department of Education Bill A592:
Prohibits Dissemination of Personal Student Information on the Internet Without
Parental Consent
The New Jersey Department of Education’s Bill A592 specifies that “…each school
district… that establishes an Internet web site, shall not disclose on that web
site any personally identifiable information about a student without receiving
prior written consent from the student’s parent or guardian on a form
developed by the Department of Education… As used in this act, ‘personally
identifiable information’ means student names, student photos, student
addresses, student e-mail, student phone numbers, and locations and times of

- Images of students that are
not recognizable (long distance, out of focus, from the back, etc.) do not
require release forms.
- Student addresses, student
email, or student phone numbers may not be published on the web.
- Student work can only be
published on the website if a signed consent form is obtained.
- Images of students taken at
public events such as sporting, or community events that are open to the
public may be used on web pages without obtaining written permission from
the students’ parents. Names or other descriptive information that
would describe these images cannot be published without written
permission.
- For all other images of
students, a signed consent form must be obtained prior to publication.

Parent/Guardian Consent Form
In accordance with
Bill A592: Dissemination of Personal Student Information on the Internet
We are sending you this parental
consent form to both inform you and to request permission for your child’s
photo/image and personally identifiable information to be published on the
district and/or school’s web site.
As you are aware, there are potential dangers associated with the
posting of personally identifiable information on a web site since global
access to the Internet does not allow us to control who may access such
information. These dangers have always
existed; however, we as schools do want to celebrate your child and his/her
work. The law requires that we ask for
your permission to use information about your child.
Pursuant to law, we will not
release any personally identifiable information without prior written consent
from you as parent or guardian. Personally identifiable information includes
student names, photo or image, residential addresses, e-mail address, phone
numbers and locations and times of class trips.
If you, as the parent or
guardian, wish to rescind this agreement, you may do so at any time in writing
by sending a letter to the principal of your child’s school and such rescission
will take effect upon receipt by the school.
